My wife's face and neck muscles tense when trying to speak?
Wow! You two have been through a lot. So, in short, I am unable to diagnose or say specifically what is going on with your wife. With that said, you have indicated that your wife has hypothyroid disease which can result in voice disorders. I would recommend seeing a Speech Pathologist that is a voice specialist.
