Cementation of the crown should only be done if the crown fits the prepared tooth well and will facilitate sealing the margins of the tooth with a suitable cement. Excess cement should be completely removed. The cement manufacturers instructions should be followed. Most replacement crowns are placed on teeth with crowns that fail because the tooth fails. The tooth develops decay that cannot be remedied without removing the crown; typically a crown cannot be removed and then reinserted. Crowns that fracture are failures due to material or technique. I have never heard of exercise being the cause of an implant failure but if you are power lifting or engaging in other exercises which might make you clench or otherwise stress your jaws there may be a slight chance that this would interfere with healing and the success of the implant.

Follow your doctor's orders, take any prescribed medications as instructed, be a bit careful of what you eat for a day or two and you will be fine. If you have a tooth extracted and an implant placed immediately in the socket with bone grafting the process will probably involve sutures (stitches) and the post operative course will require a bit more specific attention as to eating, etc. Either way, after a few days, you will feel like yourself.

This concern,however, is really lower on the scale. You should quit smoking as soon as you can at almost any cost. It is well known that smoking will shorten you life, make you less able to enjoy life, cost you a lot of money and make you and your surroundings (house, car, clothing, furniture, etc) smell bad - i.e. stink.
So instead of worrying about the effect of smoking on your implants quit smoking and improve your life.

If the individual is in constant need of dental treatment due to medical, home care or chronic illnesses issues the implant may have a shorter life. Most implants that fail do so due to biologic factors (chronic inflammation, periodontal disease, excessive biting forces, etc) rather than material factors (implant fracture, etc)
An implant does not relieve the patient of maintenance responsibility.

Fixed prostheses are attached to the remaining teeth if they are healthy or to dental implants if the patient has adequate bone for implant placement and is healthy. Attaching to teeth increases the risk of problem arising with those teeth somewhat. The best way to have a full set of teeth is to care for the ones nature gave you.
Removable prostheses (full dentures, partial dentures) are a useful method of replacing teeth and are generally less costly; they do not seal to the teeth like a dental crown and therefore allow food debris and plaque to accumulate between the denture and the teeth. This increases the risk of decay and periodontal disease affecting and damaging the teeth.
Removable prostheses are cheaper but more cumbersome; fixed prostheses are less intrusive but more expensive.
Both increase the patient's obligation for home care and regular professional care.
