This is a controversial area. I personally doubt if this disorder even exists because I see so many patients who have had the gallbladder removed for low ejection fraction and vague GI symptoms but they are no better after the surgery. The published literature on this is weak and biased toward positive results in trials conducted by surgeons who are incentivized to say that it works. Sometimes symptoms are better initially after surgery and return later. I think only a minority of cases are legitimately related to low ejection fraction. The symptom should be right upper quadrant pain after eating in the absence of gallstones, but many times the GB is removed for just nausea or epigastric discomfort, dyspeptic symptoms (with dubious results).
Vitamin b12 level?
That is a normal vit B12 level, so the gastritis is not related.

Need to know what is the cause of the pancreatitis, then future episodes can be prevented. Is it alcohol, medications, supplements, hereditary causes, etc? Get an opinion from a pancreas specialist.

The esophageal biopsy has a large number of eosinophils so this probably is diagnostic of EoE or eosinophilic esophagitis. This should be treated by your GI doctor upon follow up.

If those symptoms are secondary to the antibiotics, then they will abate after the antibiotics are stopped. However, I would guess that those symptoms may be more related to the diverticulitis itself and not the treatment. Any inflammatory process can make you feel fatigued, so with complete resolution of the diverticulitis, the fatigue should also resolve, but it will take time.
