I would be a good thought just to have a doctor visit to see why your cycles are irregular. You might want to check lab like thyroid studies, & prolactin.

Ovulation can certainly be irregular if ovulating. That in itself may affect your breast desires feeding. Also, at 41yo, your risk for miscarriage is generally higher. How long will you be breast feeding? You need to be on prenatal vitamins firing this time also. I encourage breast feeding, but cycles may be more predictable if you were not & maybe could have a better plan for the progesterone.

Birth control patch?

I would think ok if the patch is directly applied to the skin.

It could be residual from intercourse. If it is irritating, would recommend you see your Gyn doctor for a quick check to see if it needs to be treated.

The Nexplanon is a progesterone method of birth control. It works well but cycles can either stop or become erratic. I would do a home pregnancy test to be sure. The normal BC pills have both a progesterone & estrogen component that usually offers more regular cycles. I don’t dislike the Nexplanon but has advantages & disadvantages. I have a lot of patients that really like it.

I think you are on the right track. IUI may be an option too

How to treat constant burning in my mons pubis area?

It possibly could be just irritation on the mons area. It is close to the bladder area. You might want to her a urinalysis checked. A pelvic exam would be a good idea also

Urinating?

The period probably caused that. There can be uterine congestion. Be sure you are not pregnant too.

I'm spotting blood?

You should not be spitting at 64yo. You need to see your Gyn and probably have a sonogram to look at the uterine lining. It is a safety precaution.

Usually 24 hours but possibly 48 hours. Less survival the longer the interval between the sexual encounter

You mentioned she got her period. You should be ok. If irregular bleeding resumes I would recommend getting a pregnancy test to be sure. Pregnancy should be unlikely

Early adenomyosis?

Adenomyosia is more often associated with someone with multiple pregnancy history. I don’t know more info here. Adenomyosis can cause heavier bleeding
